Overview
Health-record integrity means the record remains accurate, complete, attributable, chronologically understandable, and protected from untraceable alteration.
Core Concepts
- Every entry should be attributable to the responsible author and authenticated as required by law, payer rule, accreditation standard, and organizational policy.
- Corrections and amendments should preserve the original information or an auditable history rather than erase evidence of what was previously documented.
- Late or delayed entries are not automatically invalid; their acceptability depends on transparent identification, timing, authorship, and applicable rules.
Exam Tips
- Ask: Can I tell who entered or changed this, what changed, and when?
- Never solve an integrity defect by deleting history or backdating.
