How would you distinguish an acceptable delayed entry or amendment from an integrity problem in an electronic health record?

An acceptable delayed entry or amendment is transparent. The original record remains identifiable, the added or corrected information is clearly distinguishable, and the system captures who made the change and when. I would also verify authentication and the applicable CMS, payer, and organizational requirements. An untracked overwrite, backdating, or deletion of history would undermine record integrity.

- author attribution
- date/time of change
- no backdating or hidden overwrite
- applies CMS/payer/policy requirements
Caution
A late entry is not automatically invalid. The key issue is traceability, authenticity, and compliance with the applicable documentation rule.
