Coordinated Care is the largest 2026 NCLEX-PN content area. The entry-level LPN/VN is expected to collaborate with the health care team, use credible client data in clinical decisions, contribute to the plan of care, assign or delegate appropriate tasks when authorized, process provider orders, advocate for clients, and support unbiased access to care. The safest exam answer is usually the action that keeps the nurse inside legal scope, addresses the highest client risk, communicates the right information to the right person, and preserves continuity of care.

• Assuming a supervisor or provider can authorize practice outside the nurse's legal scope.

• On every scope question ask: Is it legal, allowed by policy, within competence, and safe for this client?

Safety and Infection Prevention and Control asks whether the LPN/VN can identify a hazard before it causes harm and then apply the safest authorized intervention. The 2026 test plan specifically emphasizes equipment safety, client identification, allergies, disaster response, unsafe environments, least-restrictive restraints, timed monitoring, error/near-miss reporting, safe transfers, and infection-control principles.

• Using infection precautions only when a diagnosis has already been confirmed.

• On infection questions, identify the route of transmission before choosing PPE.

Health Promotion and Maintenance is broader than adult lifestyle teaching. The 2026 NCLEX-PN plan includes expected growth and development across the lifespan, antepartum/labor/postpartum support, newborn and pediatric care, screening, immunization needs, high-risk behavior prevention, health histories, baseline data, communication and learning barriers, expected life transitions, and community resources.

• Teaching before assessing what the client already knows.

• Assessment comes before education.

Psychosocial Integrity tests whether the nurse can recognize emotional and behavioral risk while maintaining therapeutic communication and client dignity. The 2026 plan includes emotional support, psychosocial data collection, coping, violence risk, substance misuse/withdrawal/overdose, caregiver education, group participation, cultural/spiritual/gender-identity considerations, de-escalation, cognitive impairment, stress adaptation, therapeutic communication, sensory/perceptual alterations, behavioral management, and end-of-life care.

• Using vague reassurance such as 'everything will be fine.'

• Safety first, then therapeutic exploration.

Basic Care and Comfort is about maintaining dignity, function, comfort, nutrition, elimination, mobility, skin integrity, rest, and daily living. These questions are often deceptively simple: the best answer is the intervention that meets the client's immediate basic need while preventing a predictable complication and preserving independence.

• Doing all ADLs for a client who can safely participate.

• Ask: what basic need can cause the most harm if ignored?

Pharmacological Therapies is a major PN/VN practice area in the 2026 plan: administer medications by multiple routes, reconcile medication lists, monitor blood products, calculate IV flow, count controlled substances, maintain pain devices, administer IV piggyback medications when within scope, collect data before administration, evaluate medication response, reinforce education, maintain medication safety, and perform calculations.

• Giving a medication because it is scheduled without checking the client first.

• Before every drug: order → client → assessment → calculation → administration → evaluation.

Reduction of Risk Potential is about preventing complications before they become physiologic crises. The 2026 plan includes risk identification, urinary retention assessment, diagnostic specimens, NG suction, lab monitoring, prenatal complications, point-of-care testing, vital signs, procedure-related precautions, ECGs, invasive procedures, venipuncture, compression devices, perioperative care, urinary and NG tubes, glucose monitoring, central/peripheral vascular access, focused neurologic/circulatory checks, treatment response, and reinforcement of procedural teaching.

• Treating all family-reported data as objective.

• Reduction of Risk = prevent the complication before it becomes an adaptation emergency.

Physiological Adaptation is the category where a complication is already happening or the client has an acute, chronic, or life-threatening physiologic problem that requires adaptation. The 2026 test plan expects the LPN/VN to recognize and report changes, intervene in life-threatening situations, improve respiratory status, care for tracheostomies and ventilated clients, recognize basic cardiac-monitor abnormalities, manage wounds/drains/ostomies, address fluid/electrolyte problems, and support dialysis care.
