A patient who was alert earlier is now difficult to arouse. What should a PCT/A do first, what information should be communicated, and what must not be delayed?
Strong Interview Answer
Treat the sudden decrease in responsiveness as an urgent change in condition. Stay with and protect the patient, activate the facility's emergency/rapid-response process or immediately notify the appropriate licensed clinician according to policy, and report objective facts such as the prior baseline, current responsiveness, breathing, and time of change. Do not delay escalation to diagnose the cause.
What to Listen For
- Recognizes an acute change in level of consciousness
- Immediate safety and emergency/rapid-response escalation
- Objective baseline-versus-current communication
- Does not diagnose or leave the patient unattended
