Overview
Scene size-up tests the decisions made before patient contact, when one wrong step can create additional patients. NREMT scenarios reward the EMT who pauses long enough to identify hazards, choose PPE, request resources, and approach only when the scene can be entered safely.
Core Concepts
- **Prearrival Planning:** Use dispatch details to anticipate hazards, likely patient count, PPE, staging needs, and access route. The exam expects you to update that plan when the actual scene contradicts the dispatch report.
- **Scene Safety Before Contact:** Do not start assessment in an uncontrolled environment. Traffic, violence, downed wires, fire, unstable structures, and hazmat all require scene control or staging before patient contact.
- **PPE Selection:** Match PPE to the exposure risk, not to habit. Gloves may be enough for routine contact, but blood splash, respiratory symptoms, chemicals, or unknown substances require additional barriers.
Exam Tips
- If the scenario says the scene is unsafe, the patient’s condition does not override responder safety. No EMT treatment matters if the crew becomes part of the incident.
- When a question asks what to do FIRST, scan for hazards before scanning the patient. NREMT often hides the safest answer in the opening sentence.
