Overview
Patient intake establishes a safe baseline before the provider evaluates the patient.
Core Concepts
- Use at least two patient identifiers before intake, specimen collection, medication administration, or other patient-specific care; room number is not an identifier.
- Confirm the visit purpose/chief concern and collect/update relevant medical, surgical, family, social, medication, allergy, and screening information within role and workflow.
- Normal resting adult reference ranges commonly used for orientation are pulse 60–100 bpm, respirations about 12–18/min, temperature roughly 97.7–99.1°F (36.5–37.3°C), and blood pressure roughly 90/60 to <120/80 mm Hg; age, condition, medications, activity, route, and facility reference ranges matter.
Exam Tips
- Two IDs before touching the chart, specimen, medication, or procedure.
- Technique errors are frequent exam distractors.
