Overview
The aide is often the first team member to notice a resident is different from baseline. The exam expects recognition, objective description, prompt escalation, and accurate documentation within scope.
Core Concepts
- Compare the current observation with the resident's usual baseline and care plan—not with what is 'normal' for another resident.
- Objective: measured/seen/heard facts. Subjective: what the resident says or feels. Do not turn either into a medical diagnosis.
- Report sudden, severe, or worsening changes immediately. Routine nonurgent findings follow facility reporting procedure.
Exam Tips
- Read for timing words such as sudden, new, immediately, first, best, before, after, refuses, unresponsive, or unsafe.
- Choose the answer that preserves safety and resident rights while staying inside nurse aide scope.
